Credit is already tarnished because collections are considered as R9 ratings. However, you can slightly improve your ratings by paying off the debt. It will be reported as "Paid Collections" . This will give better impact on your credit if not the best.

Check your state laws. I know in Oregon medical collections cannot be reported on your credit report. I have several and they do not show on any of my reports.
