I called the bureau myself, because my ex-husband's SSN was still listed on my credit report, as was a variation of my own that was incorrect.
I called the bureau, explained the situation, and they went through the process of eliminating those SSN's from my report with me on the phone. It took a little time, and I was on hold for about 20 minutes before someone picked up, but it was worth it - my report is now clean of all SSN's but my own.
I never had to send anything to any of the bureaus - just made a phone call. Go to their website and get the bureau's phone number and make that call!
