Letter to revoke wage assignment

A wage assignment is where you request that your employer deduct a certain amount of your wages so you can make payments to a third party or creditor. If you no longer want the wage assignment to be in effect, you need to send them a Letter to Revoke Wage Assignment. Below is a sample format of this kind of letter.
