NRS-Does anyone have an address or fax number for this company. They just called me and then my husband threatening a wage assignment and if we don't pay the pdl we have with arrowhead then we will go to court. My husband asked for the address and Tina Watkins would not give it so we could send a DV - she said they were not a collection agency. She also kept talking about a 5/10 does anyone know what that is? She was very rude and yelled. Here is the number she gave me 888-622-0222. Does anyone know a sample revoke for wage assignment I could use to give to my payroll dept?

I wrote one like this:
Date
Your name and address
The name and address of the creditor
Your loan number

To Whom it may concern:

This letter is to notify you that I revoke the wage assignment signed (date you signed). You no longer have my permission to use this wage assignment.

Make sure you keep a copy and give a copy to your payroll department.
