OK...I found an address and some info on this one...as posted before I do still owe them a small amount..

Provo, Ut.
Anyone recognize the address?? I am just wondering if they are a dba for anyone else we may know??

I'm not familiar with that particular address, but I know Arrowhead Investments and Cashnet500 dba Payday-loan-yes are based in UT.
