I am going to write them a letter of revocation of ACH debit entries to my bank account.
Would someone please, and I hate to ask this, give me a step by step, or letter outline of how to appropriately send this letter?

Make sure that you have read the contract carefully you signed with the lender. Voluntary wage assignment should be clearly mentioned in it for which you are revoking their rights. Send a copy of the letter to your payroll dept through certified mail so that you are sure about them receiving it. Once this is done, contact the company for suitable payment arrangements.

500fast cash is a scam. they gave me a deposit that i needed, okay fine, but now they are debiting money out of my account that i did not authorize. so now i have a negative balance, they did it slick though, 90 here, 30 there. im pissed
