Am I obligated to fill out a paper that the collection agency is referring to as an Information Subpoena with 10 questions about my personal information? They want bank account info, etc. They said that they have filed a judgment against me in the Superior Court.

Check with the court to see if there's actually a judgement against you. Then get back to us, either way.

Hi,
The creditor can only ask you to reply to the subpoena if they have won a jedgement against you. Hence, unclewulf is right, find out first if there is a judgement against you in the county court or not. This can very well be a plan to scam people and skim their personal information.
The information subpoenais delivered before the creditor's move to seize the personal asset of the debtor in order to compensate his/her loss.Hence, enquire properly, before filling out the information subpoena.
