Anyone have any ideas on how to write one when you have no idea on what date you agreed(totally unknowingly) to do so??? I saw one that someone had but it was to list specific dates -- which I don't have!

Hi..I am an HR rep, and I can tell you dates are not important..The only date you need is the date you want the revocation effective. All you need to do is send a letter to the creditor saying on this date you revoke any wage assignments..Then send a letter of defense to your payroll saying the same thing. A good HR/payroll rep will know that a wage assignment is always your right to revoke.

Or you can always use the phrase "On or about (date)". You shouldn't need to put the exact date, though, just the fact that you hereby revoke your wage assigment is good enough.
