Help in writing a debt verification letter

I made this mess myself so there is no use in running or trying to avoid paying. I just need a payment amount that I can live with and still eat( I make less than 500 dollars a month ). I am sure this will only be the first I will receive. I had a nice cushie,well paying job when I accrued all my credit card debt. Now, well, I am just thankful to have a job. The fact that they have filed on such a small amout does surprise me. I always thought that they would go after the bigger fish.
With all this said,could someone please help me? I can't take much more of this stress. I am scared and have no one to turn to for help other than here. I am so stupid to get myself into this mess and am ashamed as well. I really need some guidance here.

In accordance with the fdcpa, I have the right to request for a validation of my debt. This is asking for proof regarding this and verifying the same.
Please attach copies of the following with the reply:
1. The agreement which authorizes the creditor to collect debt on the alleged debt.
2. The signed agreement from the debtor conforming to pay the creditor.
3. The documents regarding the payments made on this account validating the amount.

Thanks Ashley! I didn't receive a intent to file notice,just the sheriff pulling up into the driveway. Right now I am just terrified. According to the civil summons, I have 20 days to respond, or I have to respond/file by the 3rd of Nov. I am just in one heck of a mess. I just feel so trapped with my back against the wall. Just waiting for the other shoe to fall.
