Filing a Claim

Has anyone tried to contact them? I have been getting several hundreds of dollars taken out of my acct twice a month. I am wanting to file a claim against them but do not have information to do so.

I have had this company taking funds out of my acct since june of 06, i did apply for a short term loan but since paid it off. NEver approved or signed a contract with this company.
