Florida physical address of collection agency

I was looking for a list of collections agencies. I found a state by state list but have not been able to find it again. I'm looking for a company in Florida dba Medical Revenue Services in Vero Beach. I need their physical address.
Can you help.

Here is their info pulled directly from the office of financial regulation in Florida.This dept. is their licensing agency.
