Smartest Way out of Debt

I am wondering what is the smartest way out of debt. There is debt settlement, debt consolidation loans, and debt management programs. Which is the smartest way out of debt? I have about $21K in debt and am behind on payments. I need to do something NOW, but need help doing it. Any help would be appreciated.
