2 Names but Same SS on Equifax

We went through a bankruptcy and were discharged almost two years ago. Have been straightening our information out on the credit bureaus. After the bk, though the debts are discharged, a lot of them don't add the line "discharged in bankruptcy" so just adding that line to most of my entries raised my credit score over 75 points total.
What I found was, a GREAT deal of inaccuracy in my equifax as so far, I have not been able to correct it. What it came down to was this.
Without using real names of course..
I am listed TWICE under the same social security number but two different accounts..
So I am John Doe.
My first one is listed as John Doe 5th
The second is listed as Johnathan Doe
So now, one has a bit of good credit that we have established and it doesn't show up in a real estate report. The second name has all my bad stuff in it which I have never been able to edit since my bankruptcy. So its killing my score. I mean to the tune of 75 points lower than the other 2 reporting agencies.
This not only is myself, but they have my wife split into two different accounts as well.
I have tried endless faxing, they lost the information twice. I have mailed physical proof to consolidate these two accounts into one. Nothing. I have kept records. Nothing. Each time they require 30 days in which they can fix the issue.
So sorry for the long post. My questions are, has anyone dealt with a situation like this? Is it time to get a lawyer involved? What are some recourses. It is getting tiring frankly.
Also, does anyone have a good 3 report agency that they can literally track credit reports with? I have had all 3 at the tune of $60 a month for my wife and I to monitor and dispute, but I found Equifaxs to be uselss. They charge extra just for a fico score. I have also found most programs to be biased to just one credit agency, I would like to find one monitoring all three.

I may be overstating that, but its been quite a ride because everytime you dispute, they have 30 days to clear up the issue. Just was wondering if anyone had had a credit bureau list them twice. It is myself and my wife, so together at equifax we have 4 accounts, very odd. The other two companies we cleaned our credit in about an hour each?

This looks like a case of merged credit files to me. I would send a dispute to each bureau in writing along with proof of identity and demand they correct their database. If this is ineffective,I would google the credit bureau's officers for the name of the general counsel for each bureau and send them a letter detailing the problem and asking them to intercede so you don't have to file suit over this.
