Can anyone help me with pdl laws in AZ, and forms for wage assign revocation form, plus revocation form ltr to bank to revoke automatic debit? I'm dealing with Cash Transfer Centers and time is of the essence.

"I hereby revoke any and all wage assignments I may have signed with Cash Transfer Centers."
"I Hereby revoke authorization for cash Transfer Centers, or any DBA thereof, to debit in any way, any account I may be a signer on."
