Hey, kev! All you need to do to revoke any wage assignment is a short, to the point letter. Just type the pdl information at the top. Then type the letter and say something like, "I, (state your name), revoke any and all wage assignments I have signed with your company, your partners or any affiliates." Then type your name. Done. Just be sure to keep a copy, give a copy to your HR/payroll person, and mail the pdl the hard copy certified mail. :wink:

Kev, in most circumstances, a wage revocation needs to be done before a company issues it. however, most HR people know that these are not legally binding and will not honor them. Please follow the above advice , and get your letters out asap.
