If you enroll in a program, you would make one affordable monthly payment that would be disbursed to all of your creditors every month. After paying so many consecutive months under this plan, the debt consolidation company can usually get the creditors to lower your interest rate, waive late and overlimit fees, etc.
This might be a better option for you than trying to take out a loan to consolidate your debts. If you are having trouble getting a loan at this point, if you do manage to find someone who will lend to you, chances are the interest rate will be horrible, and you'll get in even deeper.
