Where do I begin ?

Four years ago we had to have a lot of work done on our house. Knowing that our credit wasn't good, we listened to the sales person when he said that not only could he help us, he could roll all the loans into 1. Well... he lied and we were up to our ears in loans we couldn't afford.
We have been in Consumer credit counseling for 3 years and we are within a year of paying off a large chunk of debt. Unfortunately, they are not able to help us with anything that is home equity related so we still have a sizeable debt there but we are slowly paying it off.
I know we have work to do and I know we have to make a conscious effort to get things paid on time.

Well once you pay off your debt through your DS program, your debt to income ratio will be better, also after about a year your credit score should be higher, thus you may be able to refinance your house at a lower rate which might help. What about trying to sell the house and get something more affordable? Also maybe you should check out the mortgage forums, the link is up on the top, post your numbers and questions and they may be able to assist more.
Also - great job on paying your debt, just think in one year that will be gone. So you still may be able to do something with your house once you have that money freed up, applying all of it to your mortgage to get. This may help as well.
