I think it was: credit reports (although a lot of attorneys run them in their office), most recent paystub with year-to-date totals, tax returns for the past four years. For the means test, your budget doesn't really matter because they use IRS numbers based on your location and your family size. For the Good Faith test, I did have to provide monthly amounts for utilities, automobile expenses, insurance, food, etc., but they just asked for numbers, not for documentation.
