Actually, your bank will have that information. My bank contacted all of the ACH Processors on my behalf via US Mail . I typed up a C & D letter ( that I actually found a template for here) and my manager made copies and sent them out.
He wasn't allowed to give me their physical contact information, but he was kind enough to send it out for me.
