Address of Internet PDLs

I have been taken over by the 4 loan companies below to the point that I'm losing my bank account in 2 weeks and left owing the bank 1500.00 in overdraft fees. They wouldn't put an ACH hold or close the account until the fees were paid and I'm in the process of filing a Chapter 13 and cannot pay them YET. Yes, this is FIFTH THIRD, who have always been fee hungry.

Can someone tell me what the addresses or fax numbers are for the following:
Flash Loans and Integrity Advance? I'd also like to know if the following PDL companies are legal in Indiana. My attorney claims all are, but they claim they can proceed with the action that was stated on the original loan documents.

In the meantime, I also need a sample of the letter notifying them to quit calling me at work. They are also calling other numbers in the company to find me.

Also write ACh Revocations to each of these payday lenders, and give copies to your bank. Also do any Wage revocations as well. Your bank has to comply with the ACH Revocations under the Electornic Funds Transfers act.
