Settlement letter

How do the settlement letter look like? What need to be on the letter?
It can be as simple as Name of Original Creditor, Account Number
It can be as simple as Name of Original Creditor, Account Number, Current Balance, and Amount & Terms of Settlement Agreement. (IE: One payment of $200 by 4/30/09)
Always make sure that it states "agrees to accept as full settlement on this account" or something to that effect.
