Helps with response to civil summons in NC

I've been reading through the wealth of information here and want to say "thank you". I am in the process of writing a response to a civil summons in NC. I am being sued by Mann, Bracken on behalf of Target. (on my credit report this has been charged off by Target). I am within the SOL and once I file my response will be working on getting some kind of a settlement worked out (I hope).
I understand how to answer each of the numbered statements on the civil summons (i.e. agree, disagree, not enough documentation to agree or disagree). But what I don't know what format the answer should be in. I have searched to no avail to find this. I called the clerk of court who said there is no form or set format.
So.... can someone help me by telling me what information from the summons I need to include in my response? I am also planning to attach a copy of the summons to my response.
