Summons Questions

There are about 9 questions on the last page of this, and am not sure how to answer these questions without making a statement that might incriminate myself somehow.
I am presently trying to work on debt settlement with all of my cards.
I am in VA.

Is it real, I would check with the clerks office. I also received a summons from chase, but it does not have questions on it, just statements. If it is real, did they attach any evidence like the original agreement you signed when you got the card. How long do you have to file your answer? Some states you can file a Motion to Dismiss instead of an answer, you never had an agreement with Zwicker. I would try NACA and see what they say and if you have to you can check on these things with a few phone calls.
