You are correct on the ACH termination form. By law if a company receives notice that you want to stop ACH withdrawals they are required to. I would send your bank a copy; or better yet take it to them the day you mail the letter to paydayone. This will ensure you receive credit and the bank will stop the ACH on their end.

honestly, stopping the ach will not help.. i know it's difficult but closing your account is the only way to stop them. they have tons of other names to go under to get your money out of your account.
