Hello Imslori,
Are you listed on the cards? If not, then you are not responsible for paying these debts.
Yes, that's true, many of us don't know this and try to pay off the debt incurred by someone near and dear for which you may be only ethically responsible. If you haven't jointly incurred this debt, you are not responsible for paying it off.

Well Imslori, I think you should contact Medicaid and inform them about the present condition. If the facilities are having social service people, they can also assist you.
Moreover, are you having a joint account? If yes, then the accountability will be the same.

No - fortunately I'm not listed on any of her cards. She doesn't have any property/real estate either so there's pretty much nothing there even if they tried. Are there any letters or whatever we can send to her credit card companies notifying them of her condition/inability to pay?

imslori,
Do you have power of attorney (POA) on your Mom's finances? If not, just send letters to each crediter explaining her situation. I'd also suggest having phone numbers changed to perhaps your cell phone number! I'm in a similar situation with my Mom. I set up her CCs on-line and changed her number, mailing address and e-mail to mine so they wouldn't bother her. I get calls all day on my cell. In the beginning I googed all the numbers and then added the contact to my phone so the next time that number called I knew exactly who was calling. I'm going on about three months of this. They call all day every day! I keep my phone on silent at work and pick up once every couple weeks to keep them updated.
I wouldn't even consider paying the CC minimums! That would be the least of my concerns!! If your Mom has no assets, no income, there is nothing the OC can do... besides hassle you.
