what is the best way to contact credit card companies and request an alternate payment plan for 12 months? should I call them and speak to someone in their debt managment department (if one exists) or send a letter to the correspondence address?

Call them up. If you send them a letter then the chances are slim that the would reply to your mail. You would be waiting for a reply & at the same time going delinquent on your payments thus getting a negative on your credit score. I doubt whether a debt management dept even exists.. but still call them up.
