what is this bill? Im stressed over it!

I just recieved a statement that I am behind in payments for what? they want the credit card number that ends with my credit card I keep paid up. I went to TransUnion and my credit is and has been 100%, except what they show? have no idea what it is, cant find out??? my dad is 102 and my husband has two amputations and some dementia...i'm stressed, help me?

Otherwise call them and ask. And dont be giving any payments out over the phone.
