Question on Wage Assignment

When you take out the loan is it in the contract already or do they have you sign a special wsection for it? I mistakenly emptied my trash/Personal and inbox that had the 3 PDL's contracts that I signed online....uhhhg. I have 3....one from collect500, another from loanpointusa and another from some company who's name escapes me? Anyone know if they automaticaLLY HAVE WAGE ASSIGNMENTS in their contracts?
Also, if they do, I know I can send them a letter and a copy to my payroll, but how would I know what name to put on the letter when I send it to my payroll department? Some of these lenders have a few different names they can use? Is it possible to send a "Blanket" wage assignment revok form to your HR? Or I guess that would be on a company by company basis huh?

You'll need to be specific. Most lenders are adding that little

Most lenders are adding that little clause into their contracts when they provide a loan, it seems to be fairly common place anymore.
