Unsure How to Proceed (Judgment)

I called a law firm after being served papers to request a settlement amount. I agreed to pay 189 a month by the 20th of every month. I received a copy of the judgment in the mail for me to sign and send back to the law firm. I did so within a few days of receiving the paper. It states that the judgment is being fulfilled as long as I make payments by the 20th of 189. All is fine, I send in my first payment.
Two weeks later I receive mail from the law firm requesting I fill out a "financial affidavit". It is just a plan form with questions and a spot at the bottom for my signature and a notary public. NOTHING signed by a judge or anything.
Now my question is do I HAVE to fill out this affidavit? It isn't mentioned ANYWHERE in the judgment that I must fill out this affidavit and it wasn't mailed to me until after I signed the agreement, sent it back AND sent them my first payment. The affidavit includes information like "what is your bank account information?" I don't feel comfortable giving this to them. As I have heard the horror stories.
If the affidavit was court ordered wouldn't a judge have signed it and it would have come from the court house and not the law firm? I received the final copy of the judgment today with the judge's signature. Am I within my rights to continue with just making the payments as the judgment I signed states. Can they go back on the agreement if I do not fill out the affidavit? Any help is appreciated as I am not sure of my options. Thanks!
