I went to www.ftc.gov to get or fill out the affadavit report and i was unable to fill it out on line or print it. What do you suggest i do now? Is there another place to get one?

I guess you tried to fill out or print the voluntary form for reporting the fraud. You can check out the following "http://www.ftc.gov/bcp/edu/microsites/idtheft/consumers/filing-a-report.html" in order to find out a secure online version of the form which you will be able to print for your records. However, before you fill out the form, you can place fraud alert on your credit report and close those account which you think have been tampered.
