Is there a special form I need to send to the payday loan companies? What all do I need to include in the letter? And what does certify it mean ?

"I hereby revoke any and all voluntary wage assignment(s) I have

"I hereby revoke any and all voluntary wage assignment(s) I have signed with ______________(write in name of company) effective immediately."

Make sure you date it. Forward a copy to your payroll department.

Ok, Do I need to forward a copy to all the payday loan lenders as well? Can U help me get the address of all my lenders? They are Integrity advance, SSM, mycashnow, loanshop, ameriloan, and 123wages thanks for the help! Could u also find a phone number for them too?

You will need to do the legwork finding addresses. Go to their websites, should be in the contact page. Email and phone. Actually you should stay off the phone.
