I received a letter from a collector (medical bill) with threats pertaining to IRS and signed by Terri the head of the accounting. I spoke with her and discuss my hardship, told her about loosing my job. She sent me a letter with lower payment plan. In the next letter, she is now a patient advocate..

Yes, I can see what youre saying there, but to go from "head of accounting" to a patient advocate position, thats a bit of a stretch. My guess is that they lie about their title in the first letter because they want to sound more serious, and in the second, they wanted to sound more like they are on your side, working for you.
