accident in uhaul with CDW - now in collections! Help!

Hello,
I just received a call from a debt collector at NCO and I was very much surprised. Last year I moved and rented a Uhaul Cargo van and asked for the maximum insurance. Turns out they only have CDW available for the cargo vans...of course they didn't inform me of this and I rented the Uhaul thinking I was insured. I was in a small accident where the uhaul bumped into the trunk of another car. I gave him all my information to the driver and the police. I followed up with the other driver's insurance company (State Farm) after since I never heard anything. They advised me to wait as the claim had not been processed. I waited but never heard back. A year later after never receiving mail notifications I received a call from a debt collector saying I owe almost $4,000! I have no idea why I was never contacted. They are claiming that I owe the money. Is there anything I can do? Seems like it should be illegal to not offer more insurance on the uhaul. Is that really my fault? I only rented the uhaul because I do not own a car or have car insurance. What can I do in this case?
One other thing is that NCO sent me a letter stating they are requesting the state of Illinois to suspend my drivers license. Is this merely a scare tactic?

You need to request validation of the debt in writing from NCO.

Have you received anything in the mail since this call?
