Request a Credit Balance?

I tried calling them but they have an entirely automated support thing. no personnel, probably because nextcard is now gone...i think.
anyways, the nextcard website is still there and in its small FAQ section, it said that Credit Balance Requests must be done by mail.
What kind of a letter must i write? What should it contain? If they do decide to send me my money, can i ask them to route it to a different bank account?
any and all answers welcome.
thanks!
