Go to your nearest SSA Office and let them know that you've lost
Go to your nearest SSA Office and let them know that you've lost your card and they'll give you a reciept that you had applied for a new card. Hope this helps...make sure to call and ask what documents you need for proof of identity.

You will need to prove your Social Security number to the truste
You will need to prove your Social Security number to the trustee when you have your creditors meeting. Following is the list that our attorney gave us of acceptable documents: Social Security Card, Medical Insurance Card (although I don't think most of them use the SS number anymore), W-2 Form for the most recent tax year, Pay stub, IRS Form 1099, Social Security Administration Report, any other official document that has your name and Social Security number on it.
