Include a phrase in the letter that is something to the effect of:
cashing the check(s) will constitute acceptance of my offer...you agree to my payment proposal of $xxxx/month by each xxth of the month for the duration of the debt...
This is in addition to any other terms you are looking for.

CPA is a collection agency collecting on behalf of the original creditor. Try to work out arrangements with the original creditor. If your file is still in their system, they will be willing to work on your terms.
Do all the communication in writing because you need to have a written proof of all your actions. If a dispute occurs sometime later, these documents will come handy.
