My credit report

After viewing my credit report I'm a bit confused. My original cc debt to Chase in the amount of $4,343 is on here with last reported date 10/2004 and status is paid, closed/Account charged off. It also says Account closed at consumer's request. Then the very next entry is also from Chase with a different account number opened same date as first one but with last reported date 8/2005, status is closed/past due 180 days. $1,455 past due as of Aug 2005. Account closed at credit grantor's request and this one is in the amount of $9,405! I have NEVER had 2 Chase credit cards and the ONLY one I've ever had is in the amount of $4,343.
Can someone please tell me where this other account comes from? And amazingly, the $9,405 is the one that CACH says they have! I'm totally lost here!

Call Chase and verify the other account they say you owe. If the
Call Chase and verify the other account they say you owe. If they can't verify it for you, they need to get the item removed from your credit report. Put this item on dispute with the CRA and request for investigation with the companies reporting it. When CACH calls you, make them validate the account and then write a dispute letter.
