answering a summons when plaintiff has attorney

I am working on my answer to my summons. I am writing it to look exactly like the complaint with the county municipal court as heading, next I have the plaintiff's name/address followed by my name and adress as defendant. To the right where the plaintiff's had complaint I have changed that to include case number and just the title of Answers/affirmative defenses.
my question is I have Midland as Plaintiff do I need to add their attorney in the top part of my answer on the left side with the Plaintiff's name? Or do I just add them at the bottom when I attach the certificate of service that shows I sent everything to the Plaintiff's attorney?
