BMA - Civil citation advice

I was just served. It is an old BMA cc account, I think the last payment was made 12/2005. It says I need to "file a written answer to the court". Does any one here have or know where I can find in formation on how I write the letter. I have 10 days to send it in.
I am unable to pay a Attorney and not sure I will qualify with legal aid. So I need to figure out as much as I can :-(

I believe you can get the form from the courthouse and fill it in by hand. Or you can google (court documents answer to summons). You should be able to find something there. The court does expect your answer to be in a specific format, I'm pretty sure.
