Debt Verification and Health Bill

Hi,
I have been following this site and it's great info for more than four months. This site is a god send. I live in Florida.
I sent a debt verification letter that I found at this site. One CA has responded by asking me to complete an Authorization For the Use or Dosclosure of Protected Health Information. The CA is trying to collect for a supposed medical bill. The bill ask for all kind of info including my SS. Based on what I have read here, I don't think I should send them info but they should be sending me the info I requested in the debt verification letter.
Is their request legit and should I complete and return the form ?
