I would first contact the original creditor by telephone and explain the situation and see what happens. Ask them what person you should address your written correspondence regarding this matter to. Then I would definitely follow up with a written letter explaining exactly what the problem is (giving the same information as in your telephone conversation). I would also send a copy of the letter (with an introduction) to the second lender and one even to the credit bureau that this erroneous report came from.
Obviously, keep copies of everything. Someone else might have a different approach for you, but this is how I would deal with it if it were me. Good Luck!
