Three year old debt...How should I handle this??

I check my credit this afternoon and noticed 2 delinquencies appeared.

One is from feb 2006 for a walk in medial clinic which I recall paying in $50.00 cash because I was uninsured. It appears as a $200 past due collection on all three of the report bureaus. Without any saved receipts how do I get rid of this problem?

Next, there was a cable bill from Mar 2006 which i legitimately owe approximately $200.00. There were some additional charges for $250.00 that I disputed paying because the service was inactive. However, the amount in collections is $700.00. How should I handle this?

about ten years ago got credit line at local home furnishings store for $300. Never paid it regularly,went to collections,then a constable. I went to see what my credit looked like and one identity things was open-ended accts. I didn't even know what that was until it was brought up and had to find out what an open-ended acct. was. Summary: ten years ago got credit line never paid right, did collection thing. One thing I didn't tell you was about 6-7 years ago this store chain was bought by Warren Buffet's company (R.C. Wiley in Salt Lake City) How do i get this old revolving debt off my record
