Send a Dispute to the Credit reporting agencies, along with a request to remove the inaccurate info. Include a copy of your settlement letter, your payment proof, a copy of your Debt Validation letters you sent to the CA, and a copy of the CA's signature(s) on the return receipt.

I have sent 3 dispute letters but I do not have any settlement letter from 2004 as they just removed the item without letter confirmation.
