Responding to Interrogatories

I have a couple of questions I could use some insight and suggestions on. One of the first questions on the interrogatories is: State name, address, ph number, date of birth, employers address. Just how much personal information am I upheld by law to give?
On the part, that request admission, is it appropriate to answer that without sufficient documentation, I am unable to affirm or deny any such admission?
