Where do I send Check N Go ACH revocation?

I h ave read on here that in order to keep them debiting your account you should do an ACH revocation. The local manager just screams at me whenever I talk to her. What should the revocation say and where do I send it?? Thank you.
Personally I would walk in and hand it to her...have some go wth
Personally I would walk in and hand it to her...have some go wth you and do a short video on their cell phone. Then take a copy of that letter and give it to a branch manager at your bank.
