Clean Up Credit Report

I am in the process of refinance my house, just found out that there is a medical bill in my record before moved out of PA in 2004. It is a medical bill ($583, I only had the co-pay of $100, so I never expected this bill.) Collection agency is NCO FIN/51. What should I do to resolve this? Also there is an autoloan for the vehicle I have paid off with GMAC for $6900, I have already paid off the loan in 2004, sold the car, but didn't retain the paperwork. How do I clean up the credit report?

Is the Auto loan showing as a derogatory? If not, leave it alone. As far as the medical bill, I would DV NCO and when you get your green card back, dispute the tradeline on your credit reports.
