If you call a creditor and close and account because you are moving, and they dont close it and keep charging you membership fees+ late fees, which form do you send them or the credit bureau?

I dont think there is a standard form. In most cases you should be following up that the account is in fact closed. As for membership fees, did they require that you cancel in writing?
