Free Credit Report Question

Based upon a credit line decrease from Citi from information provided by Experian, I am sending Experian a letter to request that they provide me with a copy of the information given to Citi. I understand this is allowed under the FCRA, if requested within 60 days.
My question is this, what information do I have to include in the letter? Name, Address & Social Security Number? Anything else?
Also, should I put a copy of the letter from Citi in with my request to Experian, so they can verify the date or why I'm requesting the copy of my credit report? Or do I assume that they have this?
