Why did it show up twice in my credit report? Each from a different collection agency. Is it a mistake that I can dispute or there is something else I need to understand about HELOC.
In 2004, we were ableto sell the house before the default.

Only 1 CA should be able to report this however, in one situation, it is possible to have 3 tradelines reported for the same debt. If the debt has been sold to a junk debt buyer and that JDB assigns it to a CA to collect, then the OC, JDB and the CA feasibly could report. Never had this happen to me but have read a couple posts from people it has happened to. In your case, does the OC's tradeline show a $0 balance and a sold/transferred to another lender notation?
