Deperate! So many years later, now this resurfaces?

I have a secured card with BOA which is approaching the one year mark when it will then be switched from secured to unsecured. I just spoke to an agent at BOA and she informed me a negative account was added to my credit report on June 1st of this year from Verizon. It would have been for a landline.
Date Opened: 05/1999
Date Closed: 06/2012
Last Payment: 05/2001
I live in NY. I do not confirm noor deny that this debt be mine; it may be or may not be. As far as I am aware, I never owed them money. I switched out of Verizon at some point and bundled my land line with my cable provider. I have no notices from Verizon on owing them money. I just went through my files from all of the ntices I was receiving when I hit hard times; I???ve kept every one. Nothing from Verizon, nor from a collection agency attempting to collect this amont. I have not received any notices in more recent times. They simply added this to my credit report with no notification to me.
The last time I checked my FICO score it was 720. Now it???s 620.
I feel defeated; I have come so far!!!! Now what??? How do I proceed? Could someoe help my draft a letter? I have a cast on my hand and it took me so long to type this.

You need to dispute this trade line with the credit bureau. If your date of last payment was in 2001, this should've fallen off your report 7 1/2 years after that date. The "date closed 2012" is BS. It's likely a collector trying to reage the account. Dispute it.

So I should dispute this with the credit bureaus directly. What should my letter say? I don???t want to admit it???s mine, which I am not even sure it is. I really don???t remember owing Verizon anything and I haven???t found anything in my records to suggest I do.

You simply tell them that there was no activity on the account since 2001 and the recent entry is an attempt at reaging the account (illegally) and you want it removed.

Do as OhioGal suggested. The bottom line is that as far as you can tell, this is a totally inaccurate account that looks to be reporting illegally. In your letter state that you dispute the account because you do not owe verison any monies and also point out that it is past reporting age as the last payment is being reported as 11 years ago and this is looking like an illegally re-aged account.
Be prepared as there is a possibility it will be verified. If this happens, then you bring the claws out. If this happens, come back and we can help you go after them.
